Northwest Colorado Options for Long Term Care

Northwest Colorado Options for Long Term Care (NWCOLTC) is a regional program that is administered by Garfield County Department of Human Services. NWCOLTC serves the following nine counties: Garfield, Eagle, Pitkin, Rio Blanco, Moffat, Routt, Summit, Grand and Jackson.

Vision To improve access to publicly funded long term care services and facilitate utilization of appropriate services by long term care clients.

Mission To provide a single access or entry point where a current or potential long term care client can obtain long term care information, screening, assessment of need, and referral to appropriate long term care programs and case management. To allow individuals needing long term care services to live in the least restrictive environment possible and provide choices at home or in a community-based setting.
